GLP-1 medications — semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) — are genuinely effective for weight loss, and the appetite-suppression effect is often the first thing patients want to talk about at follow-up. What comes up far less often, and matters more than most people realize, is what kind of weight is actually coming off.
The scale doesn’t distinguish between fat and muscle. Your body composition does, and the difference has real consequences — for strength, for metabolic rate, and for how you feel once the weight loss phase is behind you.
How much muscle is actually at risk?
The honest answer is: less than the more alarmist headlines suggest, but not zero, and it’s worth actively managing rather than assuming it will take care of itself.
A 2026 real-world body-composition study using digital phenotyping (currently available as a preprint on medRxiv, not yet peer-reviewed — worth knowing before treating the exact percentages below as final) found that tirzepatide was associated with somewhat greater relative lean-body-mass loss than semaglutide at every timepoint measured — an “excess” lean-mass loss of roughly 1.1% at 3 months, 1.5% at 6 months, 1.3% at 9 months, and 2% at 12 months, compared with semaglutide at matched timepoints. That’s a real, measurable difference between the two drug classes, though it’s a relatively modest one in absolute terms.
Zooming out to the bigger picture: broader real-world cohort data from 2026 found that for most patients, the large majority of weight lost — on the order of 80–85% — was fat mass, and relative skeletal muscle mass was actually preserved or increased in more than 70% of patients studied. That’s genuinely reassuring. At the same time, in patients who lose 15% or more of their total body weight (which a meaningful share of GLP-1 users do), other research has found that roughly a quarter to just under a third of that total weight lost was lean mass rather than fat — a proportion worth taking seriously if you’re in that higher-weight-loss group.
Put simply: GLP-1s are not inherently muscle-wasting drugs, and for many patients lean mass is well preserved. But the risk is real enough, especially with larger total weight loss and with tirzepatide specifically, that it deserves a deliberate plan rather than an assumption that it’ll sort itself out.
What actually helps — and it’s not complicated
Two things move the needle here, and both are well-supported by the evidence: resistance training, and adequate protein intake. Neither requires a gym membership you’ll abandon in six weeks or a diet that fights against the appetite suppression the medication is already giving you.
The protein target
General research on protein and weight loss points to a range of roughly 1.2–1.6 grams of protein per kilogram of body weight per day to help preserve lean mass during a calorie deficit. If you’re also doing regular resistance training, the useful range shifts a bit higher — closer to 1.6–2.2 g/kg/day — since resistance exercise increases the amount of protein your body can actually use for muscle repair and maintenance.
The size of the effect is bigger than most people expect. One controlled comparison of a low-protein intake (around 1.0 g/kg/day) against a high-protein intake (around 2.3 g/kg/day) during a short-term calorie deficit found the low-protein group lost roughly 1.6 kg of muscle mass, while the high-protein group lost only about 0.3 kg — over the same period, on the same calorie deficit. Protein intake alone, independent of exercise, made a measurable difference to how much muscle was lost alongside the fat.
Distribution matters too: research generally supports spreading protein across meals — roughly 20–40 grams per meal — rather than eating most of your daily total in one sitting, since that pattern is more effective for stimulating muscle repair through the day.
For most people on a GLP-1, this target is genuinely the harder part, not the exercise — appetite suppression can make it difficult to eat enough of anything, protein included. Prioritizing protein-dense foods early in a meal, before appetite drops off, is a practical workaround worth discussing with your doctor or dietitian if you’re finding it hard to hit your number.
The resistance training that actually matters
You do not need to train like a bodybuilder to get most of the muscle-preservation benefit. The 2026 update to the American College of Sports Medicine’s resistance training guidelines — a synthesis of 137 systematic reviews covering more than 30,000 participants, the most evidence-dense update to these guidelines in 17 years — supports a relatively modest, consistent routine:
- Frequency: 2–3 resistance sessions per week, covering all major muscle groups.
- Volume: 2–4 sets of 8–12 repetitions per exercise for most goals.
- Intensity: a weight that feels genuinely challenging by the last 2–3 reps of each set — not to complete failure every time, but not so light that it stops being a stimulus.
- Recovery: at least 48 hours before retraining the same muscle group.
That’s it — a realistic, sustainable routine, not a second job. If you’re new to resistance training, starting with basic compound movements (squats, rows, presses — bodyweight or light-resistance versions are fine to begin with) and adding load gradually over weeks is both safer and more effective than jumping into a high-intensity program, especially if you’re also adjusting to a new medication and a lower calorie intake.

Hitting the protein target without protein powder
You don’t need supplements to reach 1.2–2.2 g/kg/day — a vegetarian or mixed Indian diet can get there with a bit of planning (see our full Whey vs Plant Protein: Which Is Better for Indians? review for a deeper dive on protein sources). Approximate protein content per typical serving, from standard nutrition composition references (values vary somewhat by source and preparation, so treat these as practical ranges, not lab-precise figures):
- Paneer: roughly 18–21g per 100g — one of the most protein-dense vegetarian staples available, comparable to chicken by weight.
- Dal (cooked): roughly 7–9g per 100g for moong, chana, or toor dal — lower than the raw/dry weight often quoted, since cooking adds water. A generous bowl (200–250g cooked) gets you meaningfully closer to target.
- Curd/dahi: roughly 7–11g per cup, higher still if strained into hung curd or Greek-style yogurt, which concentrates the protein by removing whey.
- Soya chunks: among the most concentrated options, roughly 50g per 100g dry weight — useful in smaller quantities mixed into a curry or pulao.
- Peanuts/peanut-based foods: roughly 25–26g per 100g, useful as a snack or added to a meal, though calorie-dense so worth portioning deliberately.
A realistic day built around dal, paneer or curd at more than one meal, and a soya or legume addition can reach 70–100g of protein without any powder or supplement — which comfortably covers the target range for someone in the 60–80 kg range doing regular resistance training. If you’re vegan, or eating meat/eggs, the same principle applies: spread protein-dense foods across meals rather than concentrating them in one, and talk to a dietitian if you’re finding it consistently hard to hit your number on food alone.
The one number worth tracking that isn’t the scale
The scale can’t tell you whether the weight you’re losing is fat or muscle — and on a GLP-1, it will keep dropping regardless, which makes it a poor signal for whether your muscle-preservation plan is actually working.
A more useful weekly check: are the weights or resistance levels you’re using on your main exercises holding steady or trending up, even a little, week over week? If your working weight on your key lifts is dropping alongside the number on the scale, that’s a signal you may not be eating enough protein, may be under-recovering, or may need to adjust your training — worth addressing before it compounds. If your strength is holding or improving while your weight drops, that’s a good sign your body composition is shifting favorably, even without a DEXA scan to confirm it directly.
This isn’t a validated clinical biomarker in the way a lab value is — it’s a practical proxy that costs nothing and that you can track yourself, and it directly reflects the thing you’re actually trying to protect.
Bottom Line
GLP-1 medications are effective, and for most patients, most of the weight lost is genuinely fat — muscle loss is a real but generally manageable risk, not an inevitable trade-off. The two interventions with the strongest evidence behind them are also the two you have the most control over: hitting a protein target in the 1.2–2.2 g/kg/day range (higher if you’re resistance training), and doing 2–3 sessions of resistance training a week covering your major muscle groups. Neither requires extreme measures, and both are worth building into your routine from the start of treatment rather than after you’ve already noticed strength or energy declining.
